I hereby acknowledge and agree that my team will participate in the friendly soccer match organized by Windermere Soccer Club. In consideration of being permitted to participate in this event, I, as a representative of the opposing team, agree to the following on behalf of myself and my team: Assumption of Risk: I understand that soccer is a physical sport with inherent risks, including but not limited to, collisions, falls, and injuries. I, on behalf of myself and my team, voluntarily assume all risks associated with participation in this event, including those arising from the actions of other participants and the condition of the playing field. Release of Liability: I, on behalf of myself and my team, at this moment, release, waive, discharge, and covenant not to sue Windermere Soccer Club, its officers, directors, coaches, employees, volunteers, and agents from any and all liability, claims, demands, actions, and causes of action whatsoever arising out of or related to any loss, damage, or injury, including death, that may be sustained by me, my team, or any property belonging to us while participating in the soccer match. Photo and Video Release: I, on behalf of myself and my team, grant Windermere Soccer Club the irrevocable right and permission to photograph, film, record, or otherwise capture our likeness, voice, and performance during the event. I understand that these materials may be used for promotional purposes, including but not limited to website content, social media posts, and marketing materials. Medical Treatment Authorization: In the event of injury or illness during the event, I, on behalf of myself and my team, authorize Windermere Soccer Club to seek and obtain necessary medical treatment for us. I understand that we are responsible for any medical expenses incurred as a result of such treatment. Compliance with Rules and Regulations: I, on behalf of myself and my team, agree to abide by all rules and regulations established by Windermere Soccer Club for the event, including those pertaining to player conduct, equipment, and field conditions. Acknowledgment of Understanding: I certify that I have read this release form, fully understand its terms, and voluntarily agree to be bound by them. I acknowledge that I have had the opportunity to seek independent legal advice before signing this form. *
